To begin the Quest Stranglethorn Fever, speak with Fin Fizracket (Blackwater Raiders) located at coordinates 41.86, 72.88 in Booty Bay (The Cape of Stranglethorn). You will need to seek out Witch Doctor Unbagwa and have him summon Mokk the Savage. Bring the Heart of Mokk to Fin Fizracket. To complete the quest, head to Booty Bay (The Cape of Stranglethorn) and talk to Fin Fizracket (Blackwater Raiders) at coordinates 41.86, 72.88.
Tip: to do the Quest Stranglethorn Fever, go to Mistvale Valley (The Cape of Stranglethorn), at the coordinates 50.13, 55,01, kill some Elder Mistvale Gorilla until drop the Gorilla Fang. Then, go to Spirit Den (a cave at Mistvale Valley (The Cape of Stranglethorn), at the coordinates 54.14, 46.44), talk to Witch Doctor Unbagwa and get the quest The Heart of Mokk. Will appear some gorillas to kill, wait for Mokk the Savage, kill him to drop the Heart of Mokk. Then return to Fin Fizracket (Blackwater Raiders) at Booty Bay to complete.
